First Visit (Initial Consultation)

We know you have many choices when it comes to choosing an Oral Surgeon. We thank you for selecting Dr. Lynn Pierri and the Suffolk Oral & Maxillofacial Ambulatory Surgery Facility, P.C. for your oral surgery needs. Your first appointment with our practice is an important visit for everyone including our administrative staff, Dr. Lynn Pierri and you. We use this time to gather information about you, listen to your concerns and expectations, and educate you about your condition so that you may make an informed decision.

Please note, a parent or guardian is required during the initial consultation and subsequent treatments and visits with all patients under the age of 18.

Patient registration and health questionnaire forms need to be filled out completely. Our registration form is available on this website and can be digitally submitted or printed and completed by hand. If time permits, these forms can be mailed to you at your request.

Please bring any pertinent medical condition to our attention (especially cardiac, asthma, pregnancy, etc.). State any allergies, especially to medicines. State any medicines taken, especially aspirin and herbal medic.

When you call for your appointment, please have the following information available so we may preauthorize your first visit:

  • Full name and address
  • A phone number where we may reach you
  • The name of your insurance companies
  • The name of the insured party (or employee), and their date of birth
  • Your insurance identification number(s) and group number(s)
  • Insurance claims address and phone numbers

Items that need to be brought with you at your first visit to prevent reappointment:

  • Your treatment/referral card from your dentist, physician, or orthodontist (stating procedure, tooth number, etc.)
  • Any recent X-rays you have; however, it may be necessary to take a panoramic view and/or additional specialized X-rays in order to see the position of the teeth in relation to adjacent vital structures (ie. Sinus, nerves, etc.)
  • Patient’s less than 18 years of age must be accompanied by a parent or guardian, since they cannot sign consent forms
  • A list of all current medications including aspirin and herbal medicines
  • Your medical and/or dental insurance cards and all and any necessary forms filled out and signed. If you do not have an insurance card for any union plans, you must have all applicable information including:
  • Name of Carrier
  • Insured Part name, Social Security Number and Date of birth
  • Insured ID number
  • Group number

Medical clearance
from your physician is requested as part of your treatment plan and should be faxed/brought to our office prior to treatment.

If you have a heart condition, heart murmur, mitral valve prolapse, or any artificial or prosthetic device- such as a hip or knee replacement, pins/plates in bones, pacemaker, other implants, etc.- you will need to take your antibiotic pre-medication from your DDS or MD prior to your surgical procedure or it may be prescribed by our office and/or provided at a minimal fee by our office.

Blood Thinners
If you are presently taking , i.e. Coumadin, aspirin, Ecotrin, Advil, Ibuprofen, Motrin, Aleve, vitamin E, etc. and you are having a surgical procedure performed, you may need to discontinue the medication, but only at the advice of your medical doctor. Patients on Coumadin must obtain medical clearance in writing by their physician as to whether they are an office case or hospital case and must obtain a blood test (PT/PTT) prior to undergoing surgery. It is best to discontinue herbal medicine prior to surgery, especially Vitamin E, St. John’s Wort, and Ginko.

Every effort will be made to keep our schedule running efficiently. However, due to the nature of Oral and Maxillofacial medicine; there are sometimes delays or emergency circumstances that can cause wait times beyond our control. We thank you in advance for your patience and understanding and pledge to give you the same considerations when you are seen by the doctor.

Routinely, you will be given several prescriptions to be filled out by the pharmacist ahead of time and brought with you at the procedure visit so we may better explain how to take them. Included, according to the patient’s needs, are: an antibiotic, two types of pain relievers, anti-swell medication, and a change of gauze.

Please note:you are responsible for all copayments, deductibles and non-covered procedures at the time of service.
